If you or a loved one has been diagnosed with mild, moderate, or severe dementia, you may be eligible for support through the federal government’s program, Guiding an Improved Dementia Experience (GUIDE).
Care Dimensions has been selected by the Centers for Medicare and Medicaid Services to provide GUIDE services that aim to:
Once qualified and enrolled, you’ll be assigned a dedicated care navigator who will listen to your goals and challenges. They’ll develop a support plan to meet your unique needs, offering education and support that best lines up with where you are on your journey.
A person is eligible to receive services under the GUIDE program if they meet the following criteria, as verified by Medicare:
