Dementia GUIDE Program

Supporting Patients and Caregivers

If you or a loved one has been diagnosed with mild, moderate, or severe dementia, you may be eligible for support through the federal government’s program, Guiding an Improved Dementia Experience (GUIDE).

Care Dimensions has been selected by the Centers for Medicare and Medicaid Services to provide GUIDE services that aim to:

• Improve quality of life for people living with dementia
• Reduce strain on their unpaid caregivers
• Enable people living with dementia to remain in their homes and communities as long as possible

GUIDE Program highlights

Once qualified and enrolled, you’ll be assigned a dedicated care navigator who will listen to your goals and challenges. They’ll develop a support plan to meet your unique needs, offering education and support that best lines up with where you are on your journey.

• Medicare covers these services at no additional cost to the patient and caregiver.
• For eligible participants, the program includes a $2,500 annual respite care benefit – paid for by Medicare – to alleviate caregiver stress.
• You do NOT need to change your doctor to participate in the program. Our care team coordinates with your existing health care providers.

Eligibility

A person is eligible to receive services under the GUIDE program if they meet the following criteria, as verified by Medicare:

• Confirmed diagnosis of mild, moderate, or severe dementia.
• Enrolled in Medicare Parts A and B (not enrolled in Medicare Advantage, including Special Needs Plans, or PACE programs) and has Medicare as their primary payer.
• Not enrolled in hospice. Learn more about how our hospice program can help those with dementia.
• Not living in a skilled nursing or assisted living faclity.
